Cancer Vaccine Shows Promise in Preventing Recurrence of Melanoma

Imagine beating melanoma, going through surgery and treatment, only to live with the worry that the cancer could eventually come back. For people with high-risk melanoma, that fear is very real. Now, researchers are reporting encouraging results from a personalized cancer vaccine that could help lower the chances of melanoma returning.

The treatment, called intismeran autogene, is an experimental mRNA-based cancer vaccine developed by Moderna and Merck. Unlike the vaccines we usually think about, this one is not designed to prevent you from getting cancer in the first place. Instead, it is given after melanoma has been surgically removed, with the goal of helping the immune system recognize and attack any cancer cells that may still be hiding in the body.

What makes this vaccine especially interesting is that it is personalized for each patient. Scientists analyze genetic information from the patient's tumor and identify mutations that are unique to that cancer. A vaccine is then created to teach the person's immune system to recognize those specific cancer markers. In other words, two patients may have melanoma, but their vaccines could be different because their tumors are different.

New Stuff


Researchers have been studying the vaccine in combination with Keytruda, an immunotherapy drug already used to treat melanoma. Five-year results from the Phase 2b KEYNOTE-942 study included 157 patients with high-risk melanoma that had been completely removed through surgery. Patients who received the personalized vaccine along with Keytruda had about a 49 percent lower risk of their cancer returning or dying compared with those who received Keytruda alone. The combination also reduced the risk of the cancer spreading to distant parts of the body or death by about 59 percent.

Even more encouraging is that these benefits have lasted. After about five years of follow-up, researchers continued to see improved recurrence-free survival among patients who received the combination treatment. No new safety concerns were identified during the extended follow-up.

And now there is another reason for optimism. Moderna and Merck announced in August 2026 that a much larger Phase 3 trial involving more than 1,100 patients also met its main goals. The combination significantly reduced melanoma recurrence and spread compared with Keytruda alone. However, the companies have not yet released all of the detailed Phase 3 data, so researchers will want to examine those results closely before drawing final conclusions.

This does not mean we suddenly have a vaccine that can prevent melanoma altogether. It also does not mean the treatment is ready for everyone. Intismeran is still investigational and will need regulatory review before it could become widely available.

Still, the idea behind it is exciting. Instead of treating every patient's cancer exactly the same way, doctors may eventually be able to create treatments based on the unique genetic fingerprint of an individual's tumor.

Researchers are already studying similar approaches in other cancers, including lung, bladder, kidney, pancreatic and stomach cancers.

For melanoma patients, the latest findings offer something researchers have been working toward for years, a way to train the immune system to remember a person's cancer and potentially stop it before it has the chance to return. There is still more research ahead, but personalized cancer vaccines are moving closer to becoming a real part of cancer treatment.
